Office Visit & Patient Education

Here at Texan Cardiovascular Institute your scheduled office visit includes a consultation with Dr. Nasur.

New Patient/First Time Visit

If you are coming for your first visit, our scheduling staff will provide you with the necessary forms to complete prior to your appointment. You may also download these forms from our website by clicking here. Please try to come an additional 15 minutes early for your first appointment to complete any additional paperwork that we may need and allow time for our front office to get your complete record together prior to your examination.


We ask that you bring these completed forms with you to your appointment. In addition please bring the following items:

  • Any medications you are taking including any vitamins or supplements
  • Your insurance card and any insurance referrals from your referring physician

Your First Office Visit:

Your initial appointment with Dr. Nasur will include a discussion of your medical history, a physical examination and any diagnostic testing that may be needed. Because we believe that prevention is the best medicine and education is the best tool, we will work closely with you and your primary care physician to incorporate your cardiovascular needs and general medical care.

Returning Patients

Regarding your care here at Texan Cardiovascular Institute, it is important that you keep any follow-up appointments that you are scheduled for. Dr. Nasur will tell you how often you need to have follow-up visits. The doctor will want to perform diagnostic tests as they are vital to prevention. These tests may include EKG’s, Echocardiogram, cardiac catheterization and electrophysiology studies. For these appointments, please bring:

  • All the medications you are currently taking, including any vitamins and supplements.
  • Your current or new insurance card.
